Minor Consent Release Forms

     If you are between the ages of 16 and 17 years old, you will need to get a copy of the tattoo release form and have it signed by your parents and also have it notarized. Bring yourself, your parent, and the notarized form with you for any tattoo. If you are under the age 0f 18,  and want a piercing,  you must do the same with a piercing consent form.
